What do I need to bring to my visit?

You will need to bring a valid California Identification Card - Valid California I.D. means a California Driver License or DMV issued I.D.

It is important for you to bring in any form of medical documentation regarding the diagnosis and treatment such as doctor's notes; medication bottle(s) prescript by any doctor(s)(current or previous); or copy of prescriptions; ER or urgent care discharging medical record; X-ray, MRI or CT report; injury report from any doctor or medical examiner; any physical therapist's evaluation notes or treatment plan/progress chart; copy of most recent Lab results; referral paper work from primary physician to any specialist(s) or copy of the specialist's consultation report etc. This helps our doctors with your medical marijuana evaluation and will help qualify you for the longest recommendation possible.
